Applicants for a Chapelwood Foundation Scholarship for the Fall or Spring semester are requested to read the information below before downloading the appropriate forms for submission. All information (along with requested documentation) must be submitted electronically via e-mail to the Foundation Office, tcannon@chapelwood.org, as follows.

For all applicants

For the FALL semester, we will be receiving applications starting June 1, with a receipt deadline of June 30. Once your fully completed application is received, interview dates will be established, starting as early as July 8.

For the SPRING semester, we will be receiving applications starting December 1, with a receipt deadline of December 29, a week earlier than years prior so that schools get their funds in plenty of time.

All NEW APPLICANTS will have a personal interview. An interview date will be established once the fully completed application is received. WE WILL NOT be doing personal interviews for those who have received a scholarship for the fall semester. If ALL requested information is submitted by the appropriate deadline and a grade point average of 2.5 or higher has been maintained, based on the availability of funds, and at the discretion of the scholarship committee, your scholarship will be renewed.

Chapelwood Foundation New Scholarship Applicant: This form is to be used by NEW APPLICANTS who desire to be considered for a scholarship for the upcoming semester. This form (along with other requested documents) must be accurate, complete, and submitted via email by the deadline that has been established. Do not submit this form until all requested data and information is complete. Failure to submit this information on a timely basis means that you may not be considered for a scholarship.

Chapelwood Foundation Scholarship Renewal Form: This form is to be used by students WHO HAVE RECEIVED ONE OR MORE SCHOLARSHIPS from the Foundation in the past. The student must be sure that this form (along with other requested documentation) is accurate, complete, and received by the deadline that has been established. Do not submit this form until all requested information is complete. Failure to submit this information on a timely basis means that you may not be considered for a renewal.
